I Michael Moores nästa film är det USA:s hälsoförsäkringssystem, HMO, som hamnar under luppen. Under en filmfestival i Traverse City, USA, berättade den kontroversielle filmaren att organisationen HMO redan har varnat sina anställda för att prata med honom och även givit sina anställda råd om hur de ska bete sig om de konfronteras av Moore.

Det finns flera saker som är helt obegripligt med detta. Vad jag vet är inte HMO USA:s ”hälsoförsäkringssystem”, det finns inget sådant speciellt ”system” i USA. HMO är en förkortning för ”Health Maintenance Organization”. För det andra finns det flera HMO:s i USA. Hur kan man då säga att de ”anställda” för ”organisationen” HMO har varnat sina anställda?

Hur som helst.

Man kan och bör kritisera HMO:s. Man bör känna till att HMO:s populariserades av staten genom HMO-lagen från 1973, i ett försök att hålla nere de stigande kostnaderna för sjukvården. Vad fick det för konsekvenser? Dr Richard Parker förklarar:

HMOs are government-created and taxpayer-subsidized entities that pay physicians and hospitals predetermined, per-capita fees, regardless of what medical services are actually provided. Under HMO capitation, physicians and hospitals no longer have a financial incentive to do all that is necessary to treat a patient’s illness. Rather, their incentive is to minimize the care provided. In HMOs, a physician’s pay is tied not to the services he renders, but to the services he does not render.

The essence of the HMO system is rationing. Normally, the more services a business can provide to its market, the more successful it is. That used to be true of medicine too, when doctors were paid fees for the services they provided. It is not true today. Now, the government’s ”solution” to the problem its interventions have caused is to create a perverse system under which the providers of medical care look for ways to withhold their services.

Dr Parker konstaterar därför att HMO:s inte skulle haft en chans på en fri marknad: ”Who would pay for an insurance policy that, in a medical catastrophe, arbitrarily capitates payments to physicians for life-saving services?”

Men om jag känner Michael Moore rätt lär han vinkla HMO på sitt eget speciella sätt. Jag vågar slå vad om att Michael Moore inte kommer att klandra staten för HMO-eländet. Istället lär han klandra marknaden.
